Please complete the form below with all of the relevant information. ECA will use the information supplied to lodge the completion of your professional learning in the relevant state or territory.

To ensure that we meet the required timeframes for uploading completion data towards an approved course, learners must provide evidence of completion of course within 14 days of completing the accredited course.

Please note: we can only submit requests for courses which have been completed after the course approval date.
